SNOMED International Health Declaration:

By completing the registration process for this event (whether submitted directly by you or on your behalf), you agree to the following declaration:

In attending this event I am committed to doing everything reasonably practicable in order to prevent ill health to myself and others, including:

• I will remain up to date with trusted sources i.e. local and national public health authorities at home and in host countries / municipalities, as much as possible.

• If I fit within the definition of a high risk individual, I will take the necessary precautions and guidance to protect myself.

• If I observe or present any COVID-19 symptoms prior to travel, I will follow local and national public health guidance and evaluate my continued attendance.

• I will follow any prescribed self-isolation guidance in accordance to any local and public health authorities at home and in host countries / municipalities, when applicable.

• If symptoms present at any time, I will quarantine and avoid any further physical participation until I recover.

• I am willing and able to show proof that I have completed a full course of an approved COVID vaccine, or can show proof of recent immunity or give notification of a medical exemption to SNOMED Events staff as part of the onsite check-in process and in accordance with the vaccination requirements outlined by the UK Government.

• I acknowledge that I may be required to wear a face-covering during SNOMED International Business Meeting sessions, and agree to do so if asked (unless medically exempt). 

• If I receive a positive test result during my attendance at a SNOMED International event, I am responsible for complying with applicable United Kingdom requirements. Further, should I receive a positive COVID-19 test result, I am responsible for covering any costs associated with my isolation.
